POSITION: Case Manager - Seasonal

PROGRAM/LOCATION:  Overnight Warming Locations “OWL”

SUPERVISOR: Program Supervisor


The goal of HomeFirst’s case management services at the Over Night Warming Locations is to provide access to resources for chronically homeless individuals of Santa Clara County, California. The case management services will achieve this goal by providing participants with comprehensive supportive services to help link them to a network of community resources- psychiatric, medical, employment, housing, income, and other services. 



• Maintain a case load providing light case management other direct services to clients

• Provide community-based drop-in case management services to program participants 

• Work with other team members as applicable to communicate interventions with the client and to link them to additional services

• Collaborate with community partners regarding support services for clients

• Utilize supervision appropriately, maintaining open lines of communication and providing updates on intake activity

• Know and implement agency and program policies and procedures

• Report all unusual occurrences and suspected cases of abuse immediately 

• Maintain professional relations and conduct

• Maintain thorough, accurate records of case management activities with every program participant, in the HMIS system and through a tracking system as appropriate 

• Conduct client intake per HomeFirst and County requirements, including Vulnerability Index & Service Prioritization and Decision-Making Tool (VI-SPDAT)

• Works with program participants to develop and implement an individual case management plan, and provide support to help them achieve their goals

• Meets regularly with program participants at a frequency determined by the clients’ needs

• Documents, via progress notes in HMIS, all case management activity by the end of the next business day and maintains all other case file paperwork and documentation

• Attend all meetings and trainings relevant to this position

• Assist with other duties assigned



• Bachelor’s degree + one-year minimum human service experience, or minimum of three years human services experience

• Strong written and oral communication skills

• Experience in developing relationships with community partners and outside agencies

• Ability to manage multiple projects concurrently and be flexible with priorities

• Demonstrated ability to develop and maintain priorities and meet established deadlines

• Exceptional organizational skills

• Strong sense of accountability

• Ability to present oneself and the agency professionally to internal and external audiences

• Ability to work as a member of a team and can set and meet individual goals

• Ability to work with people of diverse socio-economic backgrounds

• Understanding of and sensitivity to the needs of the homeless population

• Strong understanding of PTSD and chronic homelessness issues

• Preference to provide extensive field work and office-based work

• Enjoy providing hands-on instruction in basic living and financial skills
