ESSENTIAL DUTIES & RESPONSIBILITIES:
Assist with client and client’s family appointments and accessing needed services as identified by client and/or case manager.
Provide support to clients in completing applications and paperwork for benefits, financial assistance, housing, food, employment, child care, youth programming and mental health services within CCSCC.
Assist in Outreach and Engagement in interdepartmental activities such as distributing program brochures, locating current and potential CCSCC clients and families.
Maintaining contact with potential referral sources.
Provide assistance to finding clients “lost to care” and maintain ongoing contact with enrolled clients and families as requested.
Maintain accurate and verifiable documentation of all services rendered.
Provide telephone availability /customer service for clients during the workday.
Serve as client and their family advocate within CCSCC and neighboring community services.
Participate in staff meetings and interdepartmental /agency-wide activities.
Ensure compliance with all applicable local, State, Federal and Health Home Network regulations and agency policies.
Participate in all mandatory training.
Represent CCSCC on appropriate boards and committees as assigned.
Other duties as assigned.
Education: High school diploma or equivalent
Bilingual skills HIGHLY PREFERRED (Spanish or Korean)
Fingerprint background clearance
Provide proof of COVID-19 vaccination status
Provide valid CA Driver’s License and auto insurance per agency policy
