Housing Accessibility Modification (HAM) Coordinator

Location: San Mateo, CA Office

Hours: Full-time; $24.00 - $26.00 per hour

Supervisor: Program Manager

Job Description:


Under the supervision of the Program Manager, the Housing Accessibility Modification (HAM) Coordinator will lead and operate CID’s HAM Program.  The Housing Accessibility Modification (HAM) Program assists individuals with disabilities and older adults with the installation of ramps, handrails, grab bars, vertical lifts and other modifications to make their homes accessible. The modifications ensure that these individuals remain at home or move back into their home with greater independence.


Specific Responsibilities:


1.     Serve as primary contact to Consumers who require Home Accessibility Modifications.

2.     Conduct initial assessment and consultation with Consumers regarding HAM.

3.     Conduct intakes from consumers and agencies referring consumers, collect initial consumer data, complete/manage all required program forms, and review consumer files for City and County compliance.

4.     Serve as primary contact to grant funders of the HAM Program and with partner contractors - eVator and Home Safety Services.

5.     Manage and track HAM projects according to contract requirements for Households Served and Fiscal Year Budget.

6.     Accurately complete and maintain all electronic and paper records required for CID grant reporting.

7.     Work collaboratively as a team member with other CID staff to assist consumers in addressing issues and achieving goals.

8.     Represent CID at community meetings, task force and committees relevant to grant funding for the HAM Program.

9.     Maintain operations by following policies and procedures; report needed changes.

10.  Maintain customer confidence and protect operations by keeping information confidential.

11.  Contribute to team effort by accomplishing related tasks as needed.

12.  Provide peer counseling and Information and Referral services.

13.  Other duties as assigned.


Skills and Experience:


1.     Personal experience with individuals with disabilities and the disability community.

2.     Excellent computer skills including the ability to learn new programs on an ongoing basis.

3.     Project management and database management experience strongly preferred.

4.     Ability to communicate effectively with a broad range of people including consumers, community partners and people from diverse backgrounds.

5.     Excellent public speaking skills, the ability to present information in clear and articulate, logical and organized manner.

6.     Critical thinking and ability to develop advocacy strategies and break into manageable steps for others.

7.     BA or equivalent experience.

8.     At least 2 years’ experience in social services or advocacy setting preferably working with persons with disabilities.

9.     Bilingual-bicultural strongly preferred.
