SUMMARY

 

 

 

 

Under general supervision of the Food and Nutrition Program Manager, the Home Delivery Program Specialist performs a variety of duties assuming responsibility of Home Delivery Food Program to achieve high standards of performance, and timely delivery of food to homebound households in the City of Sunnyvale.

ESSENTIAL DUTIES & RESPONSIBILITIES

 

 

 

 

·       Assist in the development and implementation of program policies and procedure.

·       Establish effective outreach strategies to ensure population with greatest need are enrolled in appropriate programs.

·       Facilitate the Home Delivery Food Program referral and intake process.

·       Coordinate with staff to conduct ongoing monitoring of clients to determine ongoing eligibility.

·       Assists Volunteer Manager with the recruitment, training, and supervision of volunteers.

·       Monitor delivery schedules on regular basis to ensure timely deliveries and accommodate needed changes for efficiency.

·       Collaborate with Food and Nutrition staff to procure inventory needed to support Home Food Delivery Program.

·       Perform clerical tasks related to Home Delivery to include, but not limited to reports, correspondence, flyers, and forms.

·       Conduct community outreach to recruit and enroll new program participants

·       Conduct ongoing program evaluations to improve programming and client experience

·       As needed, support food and nutrition program activities

 

WORK EXPERIENCE & JOB RELATED SKILLS (Required/Preferred)

·       Must be organized and detail-oriented

·       Language Skills: Ability to read and interpret documents. Ability to communicate effectively with clients and staff. Ability to write reports, correspondence and progress notes.

·       Reasoning Ability: Ability to solve practical problems and deal with a variety of variables with a minimum of direction.

·       Computer proficiency, including Microsoft Office

·       Bilingual (Mandarin, or Spanish preferred)

·       Knowledge of treatment care resources or program development for multilingual, multicultural groups

 

Bachelor’s Degree in Social Work, human services, public health or related field and/or 2 years work-related experience
